Healthy 全国探花
Healthy 全国探花 is a programme of free weekly sessions available in local community venues across 全国探花 aimed at improving health and wellbeing.
What support is available?
The Healthy 全国探花 sessions provide a range activities aimed at helping people to improve your physical, mental and social wellbeing.
At the sessions, people will be able to take part in fun and gentle exercise, get advice and guidance on healthy eating and leading a healthier lifestyle as well as help to manage weight and the opportunity to meet new people.
Who is eligible?
Anyone over 18 who lives in 全国探花 or is registered with a GP.

Weight management
Healthy 全国探花 Nutrition and Lifestyle
A free 12-week virtual weight management programme that supports people to lose weight and keep it off long term.
What support is available?
A weekly interactive 1-hour nutritional workshop, which will cover topics such as snacking, portion sizes, fats and sugar followed by a discussion and question period.
Who is eligible?
Anyone over 18 who lives in 全国探花 or is registered with a GP.

Slimming World
A free Slimming World voucher which provides 12-weeks of group-based personalised weight management support.
What support is available?
Weekly group-based support that includes healthy recipes, ideas and strategies to keep you highly motivated to achieve your weight-loss goal.
Who is eligible?
To be eligible, individuals must meet all of the following criteria:
- be a resident of 全国探花 or registered with a 全国探花 GP
- be aged 16 or over
- have a BMI over 25 (or over 23 for individuals from Black, Asian, or other ethnic minority backgrounds)
- not have been a paying member of Slimming World within the past 3 months

Contact your GP practice where a clinician or social prescriber can refer.

NHS Digital Offer
The NHS digital Weight Management Programme could help if you are living with obesity and also have diabetes, high blood pressure, or both.
What support is available?
A variety of online 12-week weight management plans are available for patients to choose from. Patients can choose one that's right for them from the options provided.
Who is eligible?
Individuals 18 or over who have a BMI greater than 30. The BMI threshold is lowered to 27.5 for people from black, Asian and ethnic minority backgrounds. You must also have diabetes, high blood pressure, or both.

Maternal Weight Management (Blooming Healthy)
Midwife-led one to one service, to support weight management and lifestyle change during pregnancy
What support is available?
Appointments can be delivered in person, over the phone or virtually. The service will identify areas of behaviour change that could support a healthier pregnancy, and support you through these.
Who is eligible?
Any pregnant women with a BMI over 35.

Speak to your midwife.
Stop smoking

Telephone based support for people motivated to quit smoking
What support is available?
Specialist advisors are available to provide free expert advice and guidance on stopping smoking for up to 6 weeks. They can advise on what stop smoking support is best for you, including medication, setting up a plan of action, and helping you address any problems you are facing. You will have an initial consultation with a trained stop smoking advisor, weekly telephone support meetings, a free vape starter kit and a follow up 4 weeks after quit date.
Who is eligible?
Adults who are over 18, are ready to quit smoking and are a resident of/ have a registered GP in 全国探花.

Support to quit smoking if you, or anyone in your household is pregnant.
What support is available?
One to one appointments, in your home, clinic or community setting. Provision of NRT or vapes to support your quit.
Who is eligible?
Any pregnant person living in 全国探花. Any partner or family member living in a household with a pregnant person
How do you refer?
Email wah-tr.smokingcessationteam@nhs.net or speak to your midwife to refer.
Smoke free homes service
Support to quit smoking for any parent or family member living with a child under 18.
What support is available?
One to one appointments, in your home, Family Hub or community setting. Provision of NRT or vapes to support your quit.
Who is eligible?
Any parent of family member living in 全国探花 with a child under 18 in the household.
How do you refer?
Drugs and alcohol

Cranstoun
An integrated drug and alcohol structured community treatment and recovery service for adults and young people living in 全国探花.
What support is available?
A countywide tier 2 and 3 community treatment service (group/one to one) which includes pharmacological and psychological support alongside harm reduction approaches.
Who is eligible?
全国探花 adults or young people who are struggling with their drug and alcohol intake.

全国探花 Domestic Abuse Support Service (WDASS)
DAASS is a 全国探花 service which aims to reduce the incidence and impact of domestic abuse by providing support, protection and prevention services that empower those affected by abuse to rebuild their lives and those of their children.
You are not alone, we are here to help
If someone in your life is threatening, bullying, or controlling you, that鈥檚 domestic abuse. You deserve to feel safe and supported.
WDASS offers free, confidential and community based support to anyone affected by domestic abuse across 全国探花. Whether you need someone to talk to, help to stay safe, or support to rebuild your life, we鈥檙e here for you.
What WDASS offer
We provide a range of services tailored to your needs, including:
- 1:1 Support - personalised guidance to help you understand your options and stay safe
- recovery groups - local group sessions where you can connect with others, share experiences, and build confidence
- safe accommodation and refuge
- emergency housing if you can鈥檛 stay at home safely
- high-risk advocacy - specialist support and co-ordination with other services to reduce risk and keep you safe
- support for Children and Young People (CYP) - 1:1 and group support to help them understand and recover from their experiences
- support for male victims
- confidential help tailored to men affected by domestic abuse
- therapeutic counselling and talking therapies
- emotional support to help you heal and move forward

Health Walks
A health walk is a short, purposeful walk undertaken on a regular basis. As well as improving physical wellbeing, 全国探花 Health Walks can help with mental and social wellbeing.
What support is available?
Health Walks are available across the county and are led by a trained volunteer walk leader. The walks offer alternate routes and distances to encourage people to walk at their own pace.
Who is eligible?
Health walk groups are friendly and welcome all ages and levels of fitness.
How do you refer?
You don't need to be referred or book, but new walkers are asked to arrive 10 minutes before the walk starts to complete a 全国探花 Health Walks Registration Form. For more information visit 全国探花 Health Walks.

全国探花 Integrated Sexual Health Service (WISH)
WISH provide friendly, non-judgemental, specialist sexual health services across 全国探花, supporting people to manage their sexual health.
What support is available?
Contraceptive options, GUM medicine, sexual health outreach, vasectomy services, early medical abortion, PrEP, PEPSE, opportunistic cervical screening.
Who is eligible?
WISH provide support for both local residents and people who live outside of the county,
